Many members choose five times their annual income and add family protection. You may select up to a maximum amount of $250,000 per person for all certificates issued by this insurer. |

Your personal certificate will be mailed to you within a few days of your effective date. |

Yes, you may name the beneficiary for your death benefit. Other benefits, including family benefits, will be paid to you, the insured member. |

What if I decide after reviewing my insurance certificate that I don’t wish to keep it? |

Simply return the certificate within 30 days – for any reason whatsoever – and your coverage will be void and any premiums you may have paid will be fully refunded. |
